New Patients

The first step is to schedule your appointment! Once an appointment is booked, the necessary New Patient forms will be available from the Patient Portal. You will receive an email to create an account and start using this convenient portal to your forms, records, and more.

Dr. Read is fluent in English and Khmer.

New Patient Forms

Once an appointment has been booked, you will receive an email from CHARMPHR prompting you to create an account. Once you have created the account, all new patient forms will be available through that portal. You must complete all electronic forms 24 hours before your first appointment. If we do not receive these forms on time, your appointment will be rescheduled.

Please take some time to read through the consent forms before your visit, you will be asked to sign these forms at your appointment. There is no need to print out any Paperwork; the forms will be submitted electronically.

It is important to note, that Dynamic Natural Medicine is a green business and is minimizing the use of paper forms. If you do not have access to the Internet or have trouble creating an account, please give our office a call as soon as possible so we can assist you in any way we can.

Initial Consultation Fee (15 minutes): Free

Naturopathic Visit:

First Office Visit*: $65-320
Return Office visit*: $40- 225
Well-child exam*: $150-210
Adult Wellness exam*: $190-205

Acupuncture Visit:

First Office Visit*: $65-320
Return Office Visit*: $40-225
Treatment*: $80-160

* All fees based on the length and complexity of the visit. Fees do not include additional procedures, lab tests, or supplements.

Time of Service Discount

If full payment is received at time of service, a 20% discount will be given.

Non-Sufficient Funds Fee

Returned Check Fee (NSF): $30

Emergency Pager Service

If you are an established patient and need to contact the doctor on an urgent basis after office hours, you may page your physician directly at (306) 685-1649. There is a $50 charge for this service.

In case of a medical emergency, dial 911 or go directly to an Emergency Room or Urgent Care.

Cancellation Fee

A credit card will be required when booking your appointment, if your appointment is cancelled within 24 hours before your scheduled appointment, or you do not show up for an appointment, your credit card will be charged a No Show/Cancellation Fee: $50

Insurance
Dynamic Natural Medicine is currently accepting the following insurance plan(s):

• Premera
• Regence
• United Healthcare
• Washington Apple Health
• Out of Network

For all other patients, payment for services is due at the time of your appointment. Dr. Read is considered an out of network provider. If your insurance company covers the services you received, we will provide you with the appropriate paperwork to submit to your insurance company for reimbursement of your appointment. It is the patient’s responsibility to confirm coverage.
