Dr. Read is considered an out of network provider. If your insurance company covers the services you received, we will provide you with the appropriate paperwork to submit to your insurance company for reimbursement of your appointment.

It is the patient's responsibility to confirm coverage.

Once an appointment is booked, the necessary New Patient forms will be available in the Patient Portal.

Before making an annual physical appointment, check with your insurance company as to whether the visit will be covered as an annual wellness/preventative visit.

Wait for confirmation email

You will receive an email from CHARMPHR to create an account and start using this convenient portal to your forms, records, and more.

It is important to note, that Dynamic Natural Medicine is a green business and is minimizing the use of paper forms.

If you do not have access to the Internet or have trouble creating an account, please give our office a call as soon as possible so we can assist you in any way we can.

Fill out new patient forms in portal

You must complete all electronic forms 24 hours before your first appointment.

If we do not receive these forms on time, your appointment will be rescheduled.


We value the time we have set aside to see and treat you and/or your child. We do not double book appointments. If you are not able to keep an appointment, we would appreciate 48-hour notice. There is a charge based on the length of your scheduled appointment.

  • Appointments 30 minutes or less will be charged a $50 fee for missed appointments or late cancellation/rescheduling (less than 48 hours prior to appointment, Tuesday appointments require cancellation on Friday the week prior).
  • Appointments scheduled for 31 minutes or more will be charged a $100 fee for missed appointment or late cancellation/rescheduling (less than 48 hours prior to appointments, Tuesday appointments require cancellation/rescheduling Friday the week prior).

Appointment Day

Please take some time to read through the consent forms before your visit, you will be asked to sign these forms at your appointment. There is no need to print out any Paperwork; the forms will be submitted electronically.

  • If you are late for your appointment (>10 minutes), we will do our best to accommodate you. However, on certain days it may be necessary to reschedule your appointment.
  • We strive to minimize any wait time; however, emergencies do occur and will take priority over a scheduled visit. We appreciate your understanding.
